Eye Contact is locally owned and operated.  Eye Contact has two offices in Madison; on the west side near West Towne Mall and on the east side near East Towne Mall.  We also have an office in Beloit at 1035 Pleasant Street/Riverside Drive (one block north of the intersection of highways 81 and 51), and an office in Lake Geneva at 253 W. We accept EyeMed, VIPA, VSP, NVA, Unity HMO, Physician's Plus HMO as well as DeanCare HMO Point of Service plans. We assist in filing claims for several other insurance providers - call to see if we accept your insurance plan.
Regular eye examinations are an important part of maintaining your best possible vision and eye health.

If you are preparing for your first visit to one of our offices, please remember to obtain past eye records if possible, bring and/or wear your glasses or contacts (including second pairs), sunglasses, list of your current medications and please bring your current insurance card for medical visits and vision exams if applicable.
